It can be caused by anatomical factors or other reasons. However, in Korea, it can be easily treated with a minimally invasive procedure called under-eye fat repositioning. The incision is made on the inner side of the lower eyelid, using a transconjunctival approach, which means there are no visible scars or stitches to remove. While the results are not permanent, as aging may cause the problem to return, this procedure offers significantly longer-lasting effects compared to fillers or other treatments. I generally tell patients to stay away from injecting the tear trough with hyaluronic acid fillers as most of the time it leads to results that do not make the patients happy even though it may look great at first.
