Female 168cm 90kg 35yrs old 650cc UHP under the muscle I had my breast augmentation in July 2021 I wanted more upper pole and cleavage. I like the fake round form look. I feel they have dropped When I lay down they are flat and miss shaped. I’m not sure what I can have done to give me the nice round cleavage I wanted?

Good afternoon,
You do have some bottoming out and lateral displacement- and I'm sure they fall off to the sides when you lay down. I correct bottoming out and lateral displacement with the Original Internal Bra, my strong permanent internal suturing technique, which will also improve your upper pole fullness and cleavage. I first developed this technique over 20 years ago, and today it is the most common revision procedure I perform, at least 5-6 times a week- it works and it lasts!

Thank you for sharing your question and photographs.  It appears that your implant pockets have stretched, leading to malposition of the implants, especially when laying down.  A revision procedure to reduce your pocket size and perhaps synthetic mesh may be a good option for you.

Based on the pictures, you may need a breast lift because your upper pole seems flat, the implant and your own breast tissue is located in the lower pole. You can get the round look that you want with a breast lift with implants.

From your photos it appears your implants have bottomed out, likely due to the size/shape of the implant.  This can be corrected.  You can select smaller implants and tighten the capsule.   A lift may be indicated depending on the new size selected.

You had both a breast augmentation and a mastopexy (breast lift) combined.  The 650cc UHP implants are very large and heavy.  Over time, they have stretched your normal tissue and caused 'bottoming-out' which is the implant stretching the lower breast skin, elevating the nipple height and leaving too much lower pole fullness.  A revision procedure might work, but with this sized implant, re-occurrence is a definite concern.  Using ADM (Acellular Dermal Matrix) to bolster the lower breast like a sling is a definite consideration.

The size you chose is too large and heavy for your tissue and has stretched and thinned your tissue and the implants have dropped.  If you go smaller, the pockets could be tightened.

Hi and welcome to our forum!From your photos, you appear to be demonstrating lateral drift of the implant and slight bottoming out of the implants which occurs if the tissues of the chest wall were not strong to contain a larger sized implant or if the pocket created for the implant was initially too large laterally and inferiorly.Treatment consists of removal of the pericapsular tissue at the site of the lateral drift and bottoming out and "tacking down" the lateral and inferior pocket with sutures to prevent recurrence. 15% of cosmetic surgeries benefit from secondary revision.Revisit your plastic surgeon for evaluation, examination, and discussion.Best wishes...
