Upon examination, there are visible vertical bands extending from the underside of the jaw toward the neck. These are the medial borders of the platysma muscles, a paired superficial muscle in the anterior neck. With aging, the platysma often undergoes laxity and separation along the midline, which, in conjunction with overlying skin laxity, contributes to the appearance of pronounced neck bands and loss of a smooth cervicomental angle.
Correction can be achieved through a submental approach, typically involving a 3–4 cm incision placed discreetly under the chin. Through this incision, submental liposuction or direct excision of excess preplatysmal fat can be performed, followed by platysmaplasty to approximate and tighten the medial platysma borders. This technique restores tension to the muscle layer, reduces banding, and enhances the definition of the jawline and neck contour.
