Breast implants are more reliable than fat transfers when trying to achieve a specific volume. As your goals are to improve symmetry, I would favor implants.

its important to understand, no woman has perfectly symmetrical breasts. Your right breast is droopier than the left and the nipple and areola 's are at different positions. The only way to correct this is with a breast lift. Depending on how much bigger you want the breasts, this will determine whether you want a fat transfer or a breast implant. If you only want a half cup larger, fat transfers will work. Implants are more predictable and will be permanent and will give more upper pole fullness.
I highly recommend using implants as the volume retention is far more predictable than fat grafting. Depending on the technique, quality of fat, and your own body...anywhere from only 30-60% will survive. This can create problems with predictable volume and outcomes. You may need a lift on the right side as it is larger and much lower than the left side. You should see a board-eligible or board-certified plastic surgeon with extensive training in aesthetic plastic surgery of the breast.
Hi and thanks for your question. You are a good candidate for breast implant. I recommend round implant with dual plane technique for you. Round implant can give a more natural result with upper pole fullness for your case. You have not enough breast tissue, because of this I recommend dual plane technique for you. In this technique upper part of implant is covered by chest muscle and breast tissue, lower part of the implant is covered by breast tissue. Asymmetrical implant should be used for your surgery. With this combination you can get good result. This operation takes 1.5 hours and can be done under the general anesthesia. You should stay one night at the hospital to get rid of the anesthetic drugs affect. After surgery you should use sports bra and breast band for 6 weeks. Two weeks after the surgery you can start sports but you should wait for 6 weeks to use your arm strongly.
Dear 1122333,breast implant type and sizing selection is a complicated process and requires a detailed consultation that includes a discussion of your goals, a detailed examination, and a lengthy discussion. But the decision will always start and end with the patient. When it comes to sizing, we have all of our patients find 3 photos of their desired breast size. We ask for naked breast photos because clothing can obscure the true size (push up bras). Next, we consult with patients and go over the photos and take detailed breast measurements during a physical exam. Next, we using a sizing algorithm to determine a range of implants that will work based on the exam and the measurements.Lastly, during the operation, we use silicone sizers in the operating room to see how the size actually looks in the breast. We sit the patient up in the operating room, while asleep of course, and compare the sizer result with their desired look photos. If it is too small, we start the process over with a slightly larger implant. We don't stop until we get the perfect match.Of all of my breast augmentation patients, every one of them has been happy with their size due to this detailed and meticulous process. Interesting question! Implants will always give patients a more predictable and guaranteed result. This is because not all the fat that is transferred survives, and whatever survives does not have the ability to modify the breasts as much as implants do. In summary, fat grafting results are minimal and will never be comparable to implants. Some patient only need a small change and are perfect candidates, but I do not think this is your case. Different size implants might be your best option. Maybe same size implants with a small reduction and circumferential lift on your right side, if you’re ok with the scar around the areola. Both breast implants and fat grafting address volume enhancement only. Your issue is not just asymmetric breast size, but also asymmetric breast shape. You will never get symmetry without a breast lift too.
Your breasts are asymmetrical in volume, shape and position. Implants alone will not solve your problem. The technique I recommend is a mini lift. Using a circumareola incision, your breast tissue is reshaped, elevated higher on the chest wall and more medial to increase your cleavage.
