It sounds like the ideal treatment based on your photos and story would actually be a placing a smaller size implant and performing a breast lift. Placing very large implants will potentially create new problems that are rather challenging to fix. This is my professional opinion only. Best of luck!

breast implant type and sizing selection is a complicated process and requires a detailed consultation that includes a discussion of your goals, a detailed examination, and a lengthy discussion. But the decision will always start and end with the patient. When it comes to sizing, we have all of our patients find 3 photos of their desired breast size. We ask for naked breast photos because clothing can obscure the true size (push up bras). Next, we consult with patients and go over the photos and take detailed breast measurements during a physical exam. Next, we using a sizing algorithm to determine a range of implants that will work based on the exam and the measurements.Lastly, during the operation, we use silicone sizers in the operating room to see how the size actually looks in the breast. We sit the patient up in the operating room, while asleep of course, and compare the sizer result with their desired look photos. If it is too small, we start the process over with a slightly larger implant. We don't stop until we get the perfect match.Of all of my breast augmentation patients, every one of them has been happy with their size due to this detailed and meticulous process. Unfortunately it is impossible to recommend the best implant style, size or shape without an in-person examination that allows for the measurement of key breast parameters, as well as permits an in-depth discussion as to your desired goals and outcome. For your height and weight, and "narrow chest wall" 450cc is already big. If you want "fill out and get a little bigger" then you should not go with 600cc. That will look too big for your measurements. Please visit with a board-certified plastic surgeon and review before and after photos so you know what to expect when it comes to size. Good luck!

Best results are obtained when properly sized implants, based on your breast width, are used. There is always a little room to play. Larger implants are heavy and cause early drop, and displacement to the sides. Initially, large implants look attractive but as problems start arising, the excitement is replaced with frustration.

A board-certified plastic surgeon is qualified to address your issues. Board-certified plastic surgeons have spent years of training and have the top level of experience and skill. Seek out a board-certified plastic surgeon for your cosmetic needs.

Your surgeon who has examined and measured you will be the one to tell you the maximum size that will fit your frame and tissue. Just based on your one side view in a bra, 600 cc implants will be too large and heavy.

It's difficult to determine if a lift is recommended without an examination or different photos. If you would like the 600cc implants and your surgeon feels they are a good option for you, then you can proceed. The larger implant will be a bit heavier which may come into play if you are already borderline needing a lift. I encourage you to discuss your concerns with your surgeon.
