56 yr old athletic woman who has had saline implants 24yrs now. No problems with them except for the fact in the last 9 years i have developed 3 auto immune diseases. Wondering if implants are the cause. What type of extract procedure should i look for if i were to remove? En blc? Extract with complete capsole removeal? Implants are also under the muscle. I'm really confused on which way to go.

Answer: Implants There is much that is still unknown about BII. If you are happy with your implants and would like to continue having them you can elect to simply have the implants replaced. It is best for your health to remove the implants as soon as possible since you have had them for 24 years and have developed 3 autoimmune diseases. I've been performing breast implant removal from suspicious of Asia Syndrome (BII) from 4 years ago, before that, it was because of giant cells linfoma. I can attest that Asia syndrome is associated with implants, is what we call a young desease (is an autoimmune, auto inflammatory desease first diagnosed 10 years ago), but it does not happen in every patient with implants. The statistic shows that from 100 patients one can present Asia Syndrome. Not all patient develop it , 99% of the patient does not have it. Even if the prosthesis is saline, you must undergo a complete capsulectomy to avoid future inflammation. If you would like to rule out the implants as the cause or exacerbation of your autoimmune disease, I would ask for an en bloc capsulectomy, which may or may not be feasible depending on how your original surgery was performed. Typically, women who remove their implants will also require a lift with this procedure. I would also inquire about fat grafting, however if you are very athletic you may not have much in terms of donor site tissue to augment the breast following explantation.
