I had 20 year old implants removed recently after consulting with my original surgeon, and Dr. Najafian. I surprised myself that I did NOT go with the original surgeon, but the choice was easy once I met with Dr. Najafian. From the first person I met when checking in, to Dr. Najafian and Amy, my experience was full of care and compassion. I never felt self conscious or nervous, both things I was very concerned about when going into my first appointment. Your breast width is the main factor in choosing implant size. Your surgeon is right, going smaller than breast width does not give you the best result and also has the risk of implant rotation if the pocket is larger. Your other option is to chose a low profile that gives you the same width but smaller projection. However that may not give you enough augmentation you’re hoping for. Overall, medium profile 360-365 cc implant is a reasonable size for your breast width and not considered a large size.
Based on the most recent studies, there is no evidence that implants contribute to autoimmune diseases and en bloc capsulectomy resolves BII symptoms. Specially in your case with saline implant. However, with old implants, it’s important to have them checked and remove/exchange them as needed. Scientifically, the en block procedure does not offer any advantages over total or partial capsulectomy unless patient has the very rare type of textured implant related lymphoma. As a rule, I always discuss this with patients that my goal is to safely remove as much capsule as possible. However, if the capsule is very thin and healthy looking, or if it has adhesions to the chest wall (in under the muscle cases) and not looking concerning, it can be safely removed in place and just scraped. I have not had a case the capsule could not be removed but it is always important to keep that chance in mind. At the end of the day, your health and safety is your surgeon’s number one priority. Also, after having implant for a long time, they leave a footprint on the chest wall and removal of the implant without replacement or reconstruction would likely result in flat/concave appearance of the chest wall. So that goal also need to be clearly discussed with the surgeon.
The key to answer this question is how much of your own breast tissue you have. In cases of enough breast tissue (which the cut off may vary from surgeon to surgeon), the answer is yes. But if you don’t have enough breast tissue, there is a risk of compromising the blood supply by doing explant/capsulectomy and lift at the same time and staging with at least 6 months gap is recommended. I usually do Ultrasound at our office and that gives a good understanding of native breast tissue thickness and surgical planning.
Yes! Breast implants would be a good option to augment your breasts. With the shape of your breast, breast implants will open up the pocket and stretch the skin over time and improve the shape. From your photo, it seems like you have a normal nipple to IMF (inframammary fold: the fold under the breast) distance with some asymmetry in the location of the IMF the can be adjusted at the time of surgery. At the beginning, your breasts may look more round with the implant which is completely normal and expected, but over the course of months, they will settle down and take a more natural shape.
From the photos, looks like you do have enough tissue for implant placement above the muscle. I almost always do my breast augmentations in the subfascial plane which is above the muscle and under the breast tissue and rarely go under the muscle for a variety of reasons: under the muscle placement has more difficult recovery, it affects the function of pec muscle specially in physically active patients, causes animation deformity and in long term has risk of waterfall deformity. In terms of size, the main factor is your breast width, you don’t want to choose an implant that is wider or smaller that your breast width, then we can choose different projections within that width to help patients meet their goals. So, I would say if the width matched your exam, it is a reasonable size.
