Yes it is definitely possible to reduce the areolas without having implants placed. You point out another aspect of the procedure which is worth thinking about, which is the natural expansion and contraction of the areolas. If a permanent suture is used that does not occur, and this can feel unnatural. Consider use of an ADM (Strattice) donut instead.

From the photos it appears you have some constriction to the breasts. If you are not interested in implants you can consider areola reduction. This would improve the nipple herniation and downsize the areola. If you wish to address asymmetry or stretch the tissues then further surgery would be needed.

Many thanks for your query. You do have mildly tuberous breasts.In such situations I would place implants ( which helps to expand the lower pole ), and reduce the areolas in one operation. It would be possible however to just reduce the size of the areolas initially and see if you are happy with the result. If not, then you can have the implants placed and any lower pole release at a second operation.
