20 year old implants - saline- under the muscle - swishing when I run. I thought it was my lungs but believe it to be my breasts.

Although I don't advocate routinely replacing implants if there is nothing wrong with them, 20 years is an extraordinarily long time for saline implants. I have seen saline implants that appear to have lost volume over the years despite not being grossly ruptured. The valve that holds the saline inside the shell, although incredibly reliable, is not immune from failure and it's possible to lose volume over time. I would suggest consulting with a plastic surgeon to evaluate the implant. If you decide on replacement, know that replacement procedures are often much easier than the original surgery.

Sometimes if you have lost weight, the fluid in the implant can be heard when you are running since there is not as much tissue covering it as there used to be when your breasts were fuller. It would certainly be feasible to seek out consultation with a board certified plastic surgeon in your area for evaluation.
