I underwent cosmetic eyelid surgery where the doctor cut my upper eyelids and took off too much skin. I do not like the shape of my eyes now and they are asymmetrical , especially since the right side was over corrected even more. I like my old, droopy “puppy” upper eyelids. What procedures can I do to have both my eyelids closer to how they were before? Is a skin graft possible, it not what other techniques could help the situation? Thanks in advance.
Answer: This can't be repaired with a skin graft. The skin graft is too stiff and will never fold properly. You need crease lowering surgery. This is a type of upper eyelid reconstruction that involves making a new lower eyelid crease and moving the levator aponeurosis to the new crease location. A small ptosis correction should also be performed. Preaponeurotic fat needs to be mobilized from scar tissue created by the original surgery and also brought to the new crease to contribute volume to the lowered crease. You have enough skin for this. A detailed in person assessment is essential to determine what resources are left in the eyelid from the prior surgery. Your eyes can be your best feature.

Answer: Can I get my old look back? I'm sorry you are unhappy with your surgery results. Skin grafting to try to correct your previous surgery is not likely to leave you with the look you are wanting and may leave you even more unhappy. I encourage you to discuss your concerns with your surgeon for suggestions to address your concerns.

Answer: Before and after upper blepharoplasty result I do understand that your complaints are coming from your own esthetic perspective, each one of us has his/her own. However, as a board certified plastic surgeon and someone who has done hundreds of blepharoplasties, I personally think that your after result is better than your before based on these photos.

Answer: The definitive nature of surgery Many plastic surgical procedures should be considered permanent and irreversible. Upper eyelid blepharoplasty is one of them. Reconstructing upper eyelid with skin grafting is typically only done after traumatic events such as burn injuries. In uncircumcised men foreskin is the skin of choice for eyelid reconstruction. The pictures included are insufficient to make a quality assessment. From what we can see your outcome is fairly typical and regrets about having cosmetic surgery does not always justify having attempted revisions. Skin grafting that your upper eyelids will leave you looking disfigured. Accept the outcome and your choice of having the procedure and go on as is. Any attempt at revisions are more likely to leave you disappointed. If you want quality assessment, then I suggest scheduling in person consultations with highly experienced oculoplastic surgeons in your community. For second opinion, consultations come prepared bringing with you a complete set of proper before and after pictures and a copy of your previous operative report.
