42 yrs. Lost 60lbs. W127lbs and 5ft2. Im looking for a surgeon that specializes in lower body transformations. I WILL travel to get the best outcome. I have worked hard to attain and maintain this weight but my body does not reflect that. I want a very tight silhouette.Cellulite, jiggly skin, stubborn fat. What procedures would give the maximum tightness to my lower half (butt, thighs, hips, knees, calves) Can they all be combined? 1 pic is my wish. Is that a far fetched dream? Pls help!!
Answer: Best Outcome I would recommend a lower body lift, which is essentially a circumferential lower body lift with thigh liposuction and calf liposuction. You may also need a medial thigh lift procedure 3-6 months after the initial surgery because that is an independent unit that requires a single surgery on its own. I would highly recommend seeing a board eligible or board certified plastic surgeon for a physical exam and accurate analysis with safe recommendations for potential surgical options. Best of luck!

Answer: Realistic expectations It is wonderful what you have achieved after losing the weight, you must be super proud!! You must understand that tissues all over our body develop saggy skin very differently depending if the anatomical attachments to muscle fascia and bones. You have developed saggy skin on your abdomen which will be perfect for tummy tuck component and pulled the skin towards the groin are which is the fixed one. The buttocks are saggy with some signs of cellulite and that area may be lifted towards a the gluteal fascia or the lumbar fascia as well as the outer thighs which will be lifted and attached to the fascia of the hip bone but these two last (butt and outer thighs) now have a force against the lift: gravity and your tissues weight. The thicker the less the lifting, because of weight. Thighs are very tricky because circumferentially top to bottom (specially the mid third and lower third), are not attached to nothing solid but to the muscle fascia which is very flimsy, the Body lift will help to improve front and outer thighs just the upper third and the Inner Vertical Thigh tightening will only help to improve the inner thigh area but a totally 360 smooth, tight, firm thighs as your wish photos will be unrealistic expectation. Let’s first analyze your pictures and the areas that you want to be addressed. In regards to your buttock, you have excess skin and fat, and cellulite. Regarding your lateral or outer thigh, you have excess skin and fat and cellulite. Regarding your inner thighs, you have excess skin and fat and mild cellulite but mostly hanging skin. In regards to your calves, you have primarily excess fat but not too much excess skin. The best way to address the buttocks and the lateral thighs are with a 360 circumferential lower body lift. During a 360 circumferential lower body lift, the lateral thighs are lifted along with the buttocks. Excess skin and fat of the buttocks are lifted and permanently removed. If your abdomen bothers you, this can also be corrected during the same procedure; namely, the circumferential lower body lift. The body lift does not significantly impact the inner or medial thigh. To address the inner or medial thigh, you need a vertical medial thigh lift from the groin to the knee. During this procedure, liposuction is performed to remove the inner thigh fat, and the incision from the groin to the knee illuminates excess skin. Answer: Lower body contouring Based on these photos, it seems that one should consider doing your surgery in two stagers. First, an aggressive liposuction of lower extremities including calves and ankles, followed with true lower body lift 12 months later. Do not expect that this extensive surgery is going to significantly change signs of cellulitis, unfortunately. In person consultation with a board certified plastic surgeon familiar with this procedure is recommended. Good luck.
