Osteotomy - too risky? (Photo)

Osteotomies are integral part of rhinoplasty, in most patients. Nasal bones are cut and pushed together to narrow the nose and close the gap after dorsum is reduced. These are well controled procedures performed with fine osteotomes. Bones heal in new position and after approximately 3 months they are stable and fractures are healed. Althought cutting bones sounds very painfull this is pain free part of rhinoplasty. Complications after lateral osteotomies are exceptionally rare. This is an issue to be discussed with your surgeon in dateils ahead of surgery. Good luck.

Hi @lglarracas. A rhinoplasty in my opinion is one of the most difficult procedures to perform. I utilize piezosurgery in my practice. Piezosurgery is a more gentle technique that allows for greater precision. Less bruising and swelling is common with Piezosurgery. In a typical rhinoplasty, a hammer and chisel technique is performed leading to more inadvertent fractures. I hope this was helpful! Best of luck, Dr. Nima.

An Osteotomy is performed to narrow the nasal bones on the bridge of the nose. The bones are broken at the base and pushed inward. The bones heal in the new position. If you don't want a surgical procedure, fillers could be used but this is temporary. Discuss all options with your board certified plastic surgeon who will discuss various options.

Hello I am using technical piezo method so during the surgery I don't broke the nose that is why it is a better method

Everyone who desires a narrower nasal appearance requires osteotomies during rhinoplasty surgery. We employ this method regularly, and it's relatively "low risk" in the correct hands. Osteotomy is the breaking of the nasal bones. This is fine with a small chisel and then the bones are pushed toward the center narrowing the nose. You should go on some consults to get different opinions as to whether it’s worthwhile in your case.

Osteotomies are the most effective way to narrow the nasal bones. They do carry risks like any sugical procedure but can be done safely. Once the bones are broken on the sides they can be pushed inwards. The bones heal in the new position and become strong and stable in 6 to 8 weeks.

Hello. So, osteotomy involves narrowing the nasal bones. It's good to want to understand each part of your procedure, but, don't worry so much about this. This is a standard part of nose surgery that has been done for decades with good safety and outcomes. Everything has a risk, but, these are so predictable that they are regularly done.
My best advice to you is to work with a doctor you trust, who has experience, and then let her/him do what they do to get you the result you want.

Nasal bone osteotomy: osteotomy is the only way to narrow the bridge of the nose. Along with that you may wish to refine the tip and narrow or tuck the nostrils. After an osteotomy the nasal bones heal and usually do so much more quickly than bones of other parts of our body. The risk is that in rare cases one or both nasal bones may widen returning toward their former position. But this is rather rare and could be corrected if it happens.

Osteotomy is a widely used maneuver performed in rhinoplasty. This involved making small cuts in the bone usually with a very small chisel (called osteotome) or a saw. the purpose is to reshape the nasal bone and create a more concave bony definition from a more convex one. The bones will usually heal within one week, but they need to be held in place by a splint for one week otherwise the bones can widen and partially revert back to their native position. Long term complications are rare after osteotomies, but some can feel ridges and there may be some temporary discomfort in the areas.
