Osteotomy - too risky? (Photo)

Osteotomies are integral part of rhinoplasty, in most patients. Nasal bones are cut and pushed together to narrow the nose and close the gap after dorsum is reduced. These are well controled procedures performed with fine osteotomes. Bones heal in new position and after approximately 3 months they are stable and fractures are healed. Althought cutting bones sounds very painfull this is pain free part of rhinoplasty. Complications after lateral osteotomies are exceptionally rare. This is an issue to be discussed with your surgeon in dateils ahead of surgery. Good luck.

Hi @lglarracas. A rhinoplasty in my opinion is one of the most difficult procedures to perform. I utilize piezosurgery in my practice. Piezosurgery is a more gentle technique that allows for greater precision. Less bruising and swelling is common with Piezosurgery. In a typical rhinoplasty, a hammer and chisel technique is performed leading to more inadvertent fractures. I hope this was helpful! Best of luck, Dr. Nima.

An Osteotomy is performed to narrow the nasal bones on the bridge of the nose. The bones are broken at the base and pushed inward. The bones heal in the new position. If you don't want a surgical procedure, fillers could be used but this is temporary. Discuss all options with your board certified plastic surgeon who will discuss various options.

Hello I am using technical piezo method so during the surgery I don't broke the nose that is why it is a better method

Everyone who desires a narrower nasal appearance requires osteotomies during rhinoplasty surgery. We employ this method regularly, and it's relatively "low risk" in the correct hands. Hello! Osteotomies can be performed to move the bones medially, laterally, or intermediately. The actual cuts break the bone, the bone is shifted, and then the bones re-heal in the new position. Osteotomies don't have a greater complication rate over time if performed correctly. By correctly, we mean not overdone or in a manner which causes too much trauma to the nasal bones. This technique definitely helps to create narrowness but there are other ways to create narrowness such as augmenting the bridge. Thanks for the great questions.

Osteotomy is the breaking of the nasal bones. This is fine with a small chisel and then the bones are pushed toward the center narrowing the nose. You should go on some consults to get different opinions as to whether it’s worthwhile in your case.

Osteotomies are the most effective way to narrow the nasal bones. They do carry risks like any sugical procedure but can be done safely. Once the bones are broken on the sides they can be pushed inwards. The bones heal in the new position and become strong and stable in 6 to 8 weeks.

I'll keep the answer directed just towards what you've asked, but a comprehensive, in-person evaluation and discussion is certainly warranted.
Osteotomies are extremely common in nasal surgery and a routine part of most reductive rhinoplasties. Controlled incisions are made in the nasal bones which are then molded into a more favorable (i.e., narrower) position. External bandages are placed after this to stabilize the new position of the nasal bones and help to decrease swelling.
Bruising around the nose and eyes is very common when this is performed and usually is gone by 2 weeks after surgery.
There are a couple other options available for you to given the appearance of a more narrow nose such as: alar base reduction, tip-plasty, and dorsal augmentation.

Hello. So, osteotomy involves narrowing the nasal bones. It's good to want to understand each part of your procedure, but, don't worry so much about this. This is a standard part of nose surgery that has been done for decades with good safety and outcomes. Everything has a risk, but, these are so predictable that they are regularly done.
My best advice to you is to work with a doctor you trust, who has experience, and then let her/him do what they do to get you the result you want.
