That looks like a suture came out and then the wound healed over the space but hard to be completely sure.

You have what is called a double bubble. please go back to your surgeon and discuss your concerns and options.

Many times this is your old breast fold and correction of this would involve removal of excess breast skin horizontally in the fold. This probably could be done under local anesthetic in the office.

We typically don’t assess breast augmentation outcomes with arms and a stretched out position.
When you put your pectoralis muscle under tension, it can pull on the implant.
I don’t think you should have revision surgery for this unless there is a better reason to re-operate.
