Mentor Boost vs Sientra vs Motiva – which is best for cleavage + upper fullness? (Photos)

Hi everyone! I’d really appreciate some advice. My surgeon recommended 440–480cc Mentor Boost (extra high profile), but I’m still deciding between that and other options like Sientra (high profile or moderate plus), Motiva, or regular high profile implants. My starting point: mild sag and some spacing between my breasts. My goal: • fuller, round look • upper pole fullness • noticeable cleavage (even without a bra) • not too natural, but also not overly “ball” looking

In my experience, the brand is usually less important than choosing an implant width, profile, and volume that fit your breast base and soft-tissue coverage. Extra-high profile implants can add projection and upper fullness, but they do not truly move the breasts closer together; cleavage is still limited by your natural breast footprint, chest width, and starting breast spacing.
From the photo, you appear to have mild laxity and some natural spacing, so an implant alone may create fullness but not necessarily a lifted or close-cleavage look. I would be cautious about chasing 440-480 cc only for cleavage, because too much projection or a narrow implant can look rounder than desired. In-person measurements should drive the final choice.

breast implant type and sizing selection is a complicated process and requires a detailed consultation that includes a discussion of your goals, a detailed examination, and a lengthy discussion. But the decision will always start and end with the patient. 
When it comes to sizing, we have all of our patients find 3 photos of their desired breast size. We ask for naked breast photos because clothing can obscure the true size (push up bras). Next, we consult with patients and go over the photos and take detailed breast measurements during a physical exam. Next, we using a sizing algorithm to determine a range of implants that will work based on the exam and the measurements.
Lastly, during the operation, we use silicone sizers in the operating room to see how the size actually looks in the breast. We sit the patient up in the operating room, while asleep of course, and compare the sizer result with their desired look photos. If it is too small, we start the process over with a slightly larger implant. We don't stop until we get the perfect match.
Of all of my breast augmentation patients, every one of them has been happy with their size due to this detailed and meticulous process. You don't want to invest time and money into a result that is either too big or too small.
You should discuss your concerns with a board-certified plastic surgeon and make sure that your surgeon understands your goals.

Achieving that "not too natural, but not overly ball-looking" result requires a careful balance between projection and base width, and your surgeon’s recommendation for the Mentor Boost (extra high profile) is likely aimed at overcoming your mild sagging to give you that "pop" on top. While extra-high profiles are excellent for upper pole fullness and cleavage, they can sometimes look a bit narrow if your chest wall is broad, which might emphasize the spacing between your breasts rather than closing it. If you want a rounder look that bridges the gap for better cleavage without a bra, you might also consider a high profile (rather than extra high) in a slightly higher volume, such as a Motiva Full or a Sientra High Profile. These will give you a wider "footprint" to bring the medial edges closer together while still providing the projection needed to fill out your existing tissue. Ultimately, because you have a bit of skin laxity, the firmer "Boost" or a cohesive silicone gel will hold its shape better against gravity, ensuring you get that roundness you’re after without the implants bottoming out over time.

Hello! I would recommend a Hybrid breast augmentation which is breast augmentation with  fat grafting for better cleavage and upper pole fullness. You seem to have lower breast foot print, so even after just breast augmentation the upper pole fullness will be lacking. To achieve that my personal recommendation would be a hybrid breast augmentation.  It will help you achieve more youthful breasts.

Based on your goals—fuller, rounder breasts with more upper fullness and cleavage—I would recommend Motiva round implants.

These are newer-generation implants that provide a beautiful, natural feel while still giving that lifted, fuller look in the upper breast. They also have a very low complication rate and can often be placed without going under the muscle, which typically means less discomfort and a quicker recovery.

Overall, Motiva round implants are a great option to achieve the balance you're looking for—enhanced, but not overly artificial.

It is important for patients to understand that augmentation will "augment" all aspects of your anatomy. The spacing of the footprints of the breast on the chest wall will vary from one individual to the next. Widely spaced breast will appear more widely spaced and vice versa. 
The spacing (cleavage) is defined by the pocket. The pocket is defined by the plane of augmentation. When placed submuscular, the medial/middle limit is the sternal origin of the muscle. Wide sternum  greater separation of the breasts  less dramatic cleavage. This space can be narrowed (with some risk of synmastia) when the devices are placed subglandular or "over" the muscle. There is no muscle obstructing a medial push, however, there is a higher risk of over-dissection and midline migration of the implant (the dreaded "uni-boob").
With regards to your specific question, a higher profile device is narrower but will project more. A wider device can create the illusion of closer spacing between the breasts (when placed subglandular). Most patients will fit into the "moderate +" category...but this will depend upon the measurements of your breast width. That being said, it is never recommended that you stray far from your unique BWD.
With regards to your specific question, implants will not create cleavage but a device with a wider base diameter will provide greater upper pole fullness. Lower projection and submuscular placement can help avoid the "ball" looking effect.
And remember...love the look you choose...not the cup you desire.

Thank you for your question.  I have been in practice for over 30 years, and breast surgery and revisionary breast surgery is a large portion of my practice.  Your anatomy is a very specific type of anatomy in which you have low hanging breast or slide down of your chest/breast tissues on your chest.  In your case, performing just a breast augmentation will not give you a good aesthetic result.  You will definitely need implants and a lift.  You should be able to have a beautiful result.
