Considering Breast Aug/lift Revision - Would A Different Profile Offer More Base Diameter? (Photos)

Question

33y/o female. 125 lbs, height 5'6.5 I had breast implants done Febuary 2025. I consulted several surgeons. Some said I needed a lift, while most said I did not, as my aerola was flush with my breast fold. Some said it was my decision, as I was on the border, or had low ptsosis. I opted not to get the lift, as I wanted larger implants, and dr said larger implants will give me the lift I need. I ended up getting 550 cc high profile round silk surface plus MOTIVA. I am not completely in love with the results. I wanted more wide implants, with more "side breast" width. I find them to be too projected, and too narrow. My natural breasts were more wide, and I feel like the side of my breasts now have excess skin on the sides, as they could have been more filled out. I also do not like where the breast is sitting on my chest wall. I find they are sitting too low, not in a sense that they are sagging, but the implant itself feels like it is placed low on the chest wall. Is this fixable? I dont want to have another surgery if I am being too picky, and this is just my natural anatomy. My question being, are my concerns justified in having another surgery? I was thinking about doing same cc - 450 -550 cc but with a different proflie, either low or moderate so I get more base diameter. It is hard to find clothes and bras because they are so narrow and projected. I also want them to sit higer on my chest wall.

I would separate two issues: implant dimensions and implant position. A lower or moderate-profile implant of the same volume usually has a wider base and less projection than a high-profile implant, so it can sometimes create a broader look. But the correct choice depends on your measured breast width, ribcage, soft-tissue coverage, and pocket limits. Going wider than your anatomy allows can create side heaviness, implant visibility, or malposition rather than a better shape.
If the implant feels too low on the chest wall, changing profile alone may not fix that. A revision may need pocket adjustment, inframammary fold control, internal support/capsulorrhaphy, implant exchange, and possibly a lift if the nipple or skin envelope still needs repositioning. Your concerns are reasonable enough to discuss in person, but I would plan revision from measurements and exam findings rather than choosing 450-550 cc and a profile first.

Dear jxal,

breast implant type and sizing selection is a complicated process and requires a detailed consultation that includes a discussion of your goals, a detailed examination, and a lengthy discussion. But the decision will always start and end with the patient. 
When it comes to sizing, we have all of our patients find 3 photos of their desired breast size. We ask for naked breast photos because clothing can obscure the true size (push up bras). Next, we consult with patients and go over the photos and take detailed breast measurements during a physical exam. Next, we using a sizing algorithm to determine a range of implants that will work based on the exam and the measurements.
Lastly, during the operation, we use silicone sizers in the operating room to see how the size actually looks in the breast. We sit the patient up in the operating room, while asleep of course, and compare the sizer result with their desired look photos. If it is too small, we start the process over with a slightly larger implant. We don't stop until we get the perfect match.
Of all of my breast augmentation patients, every one of them has been happy with their size due to this detailed and meticulous process. You don't want to invest time and money into a result that is either too big or too small.
You should discuss your concerns with a board-certified plastic surgeon and make sure that your surgeon understands your goals.

As one moves up in profile, volumes and projection will increase for a give base width. Higher profile devices of the same width will generally be higher volume. This greater volume, in turn, result in greater projection.

For a given volume, a lower profile device will be wider (i.e. higher base diameter). The effect of this greater diameter is a lower profile. The greater base diameter can be utilized to achieve greater upper pole fullness.

With regards to your specific question, improving the position of low sitting breast as it relates to the chest wall necessitates a formal lift. This comes with a greater scar burden but will improve the positioning of the breast mound on the chest and address some of your concerns.

Your concerns are valid and appear to be related primarily to implant selection and pocket positioning rather than an abnormal complication. High-profile implants tend to create more projection with a narrower base, which can explain the lack of side fullness and the “too projected” appearance you’re noticing. Additionally, the impression that the implants are sitting low may be related to pocket characteristics or mild tissue laxity rather than true sagging. These issues can often be improved with revision surgery by selecting a wider, lower- or moderate-profile implant to better match your chest width, along with pocket adjustment or internal support to optimize positioning. While anatomy plays a role, a well-planned revision can significantly enhance balance, width, and overall natural appearance.

Low profile wide diameter implants will provide some improvement in shape, producing better fill in upper and lateral breast. If you can accept the scars of a breast lift, the low position of the implant would be improved as well.

Thank you for sharing your photos. Looking at your anatomy, you have what we call a 'low breast footprint,' meaning your breast tissue naturally sits lower on the chest wall. Because of the sagging in the lower poles, a lift-augmentation combination is often the ideal approach here.
Your implants actually provide good lateral coverage, so I wouldn’t worry about the width. However, to soften that sharp transition at the top and create a more natural, youthful slope, fat grafting is an excellent tool. It ‘camouflages’ the implant edge and restores that upper-pole fullness you're looking for.

It seems, based on your preoperative photos, that you had a low sitting breasts; once your breasts were augmented with implants, this became more visible?  Replacement of the implants, breast lift with creation of a new IMF, would be my suggestion. Good luck.

Breast shape is determined by the volume of fat and breast tissue and the skin brassiere. The nipple position relative to the inframammary crease also has to be taken in to account. 
When deciding what implant to use we determine first the overall size of breast you desire (done with sizers in a bra), then we select the implant profile based on your base width (measurement) then determine if a lift is necessary
Your photos appear to show large implants in a sub glandular position. This leaves the weight of the implant on the skin alone even if placed “sub facially”. There appears to have been skin stretching as well.
I believe you would be best served with going to an implant that fits your base dimension, converting to a sub muscular pocket and possibly doing a lift. I do not believe just going to a smaller implant with a wider base will get you the shape you desire.

Looking at your pre-op and post-op photos, what you actually needed alongside the augmentation was a lift.
Your breast position was already sitting relatively low on the chest wall before surgery. The footprint itself was low-set. Adding a larger implant has weighted the breast further downward, which is why things look lower now rather than more centred.
Your concerns about the narrow projection and lack of side fullness make complete sense. But changing the profile alone is unlikely to fix this.
The current implant already occupies the available width of your breast tissue. Going to a lower profile would increase the base diameter, but there isn't enough breast tissue beyond the current footprint to support that change in any meaningful way.
What would genuinely help is a lift combined with a slight reduction in implant size. Maybe 50 to 100cc smaller.
Once the breast is repositioned higher on the chest wall, the lift itself contributes to the fullness and shape. So even with a smaller implant, the overall result can look very similar in volume, just far better in proportion and position.
This is exactly how I approach it with many of my breast augmentation surgery patients in India. The lift and the implant size are planned together from the start, because one without the other rarely gives you the result you're actually looking for.
