Is 550-600 cc big for sub-glandular procedure?

Question

I am not finding many b4/after with silicone mod profile 550-600 cc above the muscle. My Brd Cert PS has done 10,000 BAs says I have plenty of tissue to cvr the implant, he measured me & I tried on sizers. Which seemed big, but not bad in bra. I am worried about how they will look. I have sag, but not enuf to do a lift. I don't want the ball in sock look, or to be huge. I am going for proportional and fuller. I'm 44 yo. 5'8, and 160 lbs. Are there cons with this size in conjunction with this placement.

anatomy and an examination.  Make sure you understand the benefits and risks associated with sub glandular implants.  If your unsure, have a second consultation with a board certified Plastic

Thank you for your question.  That is definitely a large implant.  I tend to not put implants that large above the muscle for a couple of reasons. There will be more capsular contracture (scar tissue) when placing the implant above the muscle.  Also, such a large implant over time will stretch the skin and even more importantly the pressure of the implant will cause loss of breast tissue volume over several years.  All this put together means the implants will not last quite as long above the muscle as below.

When I place 600cc implants, my patients are asking me to make them look augmented. If you liked them in a sports bra, you may be happy with that large of an implant. But you said that you don't want to be huge.I think there are some real advantages of going behind the muscle, so please consider that. You will have a more natural appearance with a decreased risk of capsular contracture, and a better mammogram in the future. I'm impressed if your surgeon has done 10,000 augmentations. That would be two a day, every day for 20 years. Best of luck!

Hello, implants of this size might appear very large in a person with a petite body frame, but might look proportional in a person with a fuller figure.  It sounds as though you had a thorough consultation and your surgeon is very experienced so their advice will be more valuable than anything you can get based on your description alone.  That stated, there is no harm in getting another consultation if you want to ensure you're making the right choice.

Yes, this is quite a large, wide implant, even for someone who is tall (5'8).  It's unlikely to look proportional, IMO, and instead look large/busty for most people.  So, like you, I have some concerns about this selection, unless you are looking for a DD+ size.If you would post some measurements and photos, this would help to evaluate your situation better.  Perhaps you should get a second opinion.

I do not perform on top of the muscle augmentation for the last 10 years, the reason being, after 2-3 years a lot of these implants start to sag really bad, the bigger the implant the more this happen. You need to see patient with implants place longer than 2 years. What you are afraid of, rock on sock look, is something that likely will happen.be sure you seek the help of a member of The American Society for Aesthetic Plastic Surgery, this will assure of the proper training and commitment to plastic surgery from your surgeon

It's difficult to give you perfect online advice without examining you in person.  However, I would say that a 550-600 cc implant is quite large.  You may consider getting a second opinion.  Be sure to clearly describe your goals.  You seem to have a good concept of your desired outcome.  Find pictures of your desired look and bring them with you. Thank you for the question.  Online consultants will not be able to provide you with specific enough advice to be truly helpful. Ultimately, careful communication of your goals (in my practice I prefer the use of goal pictures, direct examination/communication in front of a full-length mirror, and computer imaging) will be critical. 
Generally speaking, the best online advice I can give to ladies who are considering breast augmentation surgery ( regarding breast implant size/profile selection) is:
1. Concentrate on choosing your plastic surgeon carefully. Concentrate on appropriate training, certification, and the ability of the plastic surgeon to achieve the results you are looking for. Ask to see lots of examples of his/her work.
2. Have a full discussion and communication regarding your desired goals with your plastic surgeon. This communication will be critical in determining breast implant size/type/profile will most likely help achieve your goals. 
In my practice, the use of photographs of “goal” pictures (and breasts that are too big or too small) is very helpful. For example, I have found that the use of know words such as “natural” or "C or D cup” or "huge" etc means different things to different people and therefore prove unhelpful.
Also, as you know, cup size varies depending on him who makes the bra; therefore, discussing desired cup size may also be inaccurate. Again, the use of computer imaging has been very helpful during the communication process, in our practice.
3. Once you feel you have communicated your goals clearly, allow your plastic surgeon to use his/her years of experience/judgment to choose the breast implant size/profile that will best meet your goals. Again, in my practice, this decision is usually made during surgery, after the use of temporary intraoperative sizers.

Yes, absolutely 550-600cc breast implants are very large. Your breast base width measurement which your surgeon should have measured is an important guideline for choosing the appropriate size. You need to ask your surgeon what this measurement is and check the breast implant sizing chart for the appropriate size for your breast.The busiest breast implant surgeons usually due at most 200 breast augmentations a year in most cases. Has your surgeon been in practice for 50 years which it might well take to do 10,000!
