This is a very challenging problem to fix. One option is to reconstruct the lower part of your breast/chest wall and reinforce it with a mesh (internal bra) while opening the breast pocket higher up. Going up to a bigger implant will be difficult, and in fact down-sizing your implant may help with the overall shape and position of your nipples on your breast.

This is a complex breast problem. This can happen for numerous reasons including selecting the incorrect size breast implant, poor technique, poor scarring, poor healing, and/or a combination of the previous. I would recommend a revisionary breast procedure to downsize to a more appropriate implant, place an internal bra for support (Galaflex), and then another lift.

It should be possible to achieve the appearance of lower nipples by either raising the implant positions with a capsulorraphy, revising the mastopexies to shorten the distance between your nipple and the inframammary folds, or both. To determine the best option would require a physical examination. Discuss your options with your surgeon first, then seek another opinion with a board-certified plastic surgeon if necessary.

Thank you for your question. A large portion of my practice is dedicated to breast revision surgery. Unfortunately, you definitely had a poorly performed breast lift. I have fixed similar issues in the past. I have found the best way to address this is to elevate the breast implant pocket by performing an internal bra-capsulorrhaphy. You may have to accept a more round appearing breast to center the areolas. Best wishes to you.

What you actually need to have done at this point is a wedge excision of the inferior pole with a slightly smaller implant to bring the nipple position down

The inferior pockets can be tightened and raised to let the areolae be better positioned. You might need mesh for support.

You cannot move the nipples down as this will leave a scar.
It is best to open the pocket superiorly and elevate the fold with mesh fixation

The solution will require implant removal and lower pole tightening.

Lowering the nipple areola complex one’s positioned too high is difficult. If the implants have bottomed out that it makes the condition worse. Reinforcing the implant and treating the bottom. Doubt appearance is one option. Formerly lowering the nipple areola complex is very difficult.
