I always use a medpor implant with screwfixation, which stays in place over time. I custom fit this for each patient's bone structure. A silicone cheek implant has ahigh rate of malrotation over time. Makesure you specifically look at before and after pictures of real patientswho have had this surgery performed by your surgeon and not just a computeranimation system. The most important aspect is to find asurgeon you are comfortable with. Implants are a wonderful option for improving width and providing structure. I recommend cheek implants for patients that have a bone deficiency or seeking permanent options for cheek augmentation. This is the only option that will create a permanent result and will be cost effective overtime. The value of custom cheek implants is that they create augmentation effects that exceed what standard cheek implants can do. This is most manifest in the zygomatic arch area, both in extension and width. This may be what you are referring to by adding structure to the uoper face.
