Can I get a filler injected to further lengthen my columella & if I have a silicone implant in it?

You have had good success lowering a slightly retracted hanging columella with microdroplet Silikon-1000 injections. We have not had any issues in those patients with a personal history of a silicone nasal implant. Please consider re-asking your question with photographs for a more descriptive answer. Thank you. I would not suggest injecting your columella if there is a silicone implant in place at this time.  Silicone implants that support the nasal tip concern me.  The fact that you have had no issues with the implant in 11 years is fantastic.  Injecting near the implant or disturbing the area of the implant with the potential of introducing infection into the area of the implant with filler is something that I would advise against if all you are trying to improve is columella show. 

You really need to consider fixing your problem definitively with a rib cartilage graft which can replace your silicone implant and provide you a permanent solution. Inevitably, you silicone implant will wear through your nasal soft tissues and become infected. A rib graft can replace the entire implant and also help extend your nasal tip and provide structural support of your nasal tip. Fortunately, today, you can obtain an organ donated rib graft that avoids the need to harvest your rib. A consultation with a revision nasal specialist will provide you a more definitive recommendation.

I Inject a lot of noses. You can certainly fill in clefts in the columella, and what you are requesting is similar to this. The way I approach injections is to first inject some local anesthetic into the area which duplicate the effect of the filler. That way we can both look at it and decide if it works before injecting product.
