400cc high profile implants too large for my petite frame. I want to be able to fill a bikini top and get a nice cleavage. 6 years on and having breastfed two little ones for a combined total of 2 and a half years it's time for a change. I would like to completely remove the implants (submuscular), have a lift and transfer excess fat to my breasts to get a little more fullness. I don't mind small breasts but don't want disfigured, wrinkly, deflated breasts. Any advice would be much appreciated
Answer: Implant removal Due to your petite size you will want to have a detailed discussion with your surgeon about your goals. It may be that a small implant is a better choice as you may not have enough fat to harvest depending on the volume you are desiring after surgery. See a board certified plastic surgeon in your area to learn more.

Answer: Removal Yes, I would do this in stages: Removal and lift, followed by fat. I recommend an in-office examination as well as a detailed discussion with a surgeon who you are comfortable with. Finally, make sure your surgeon is a Double-Board Certified Plastic Surgeon certified by the American Board of Plastic Surgery (ABPS), a member of the American Society of Plastic Surgery (ASPS), a member of the American Society of Aesthetic Plastic Surgery (ASAPS), and a fellow of the American College of Surgeons (FACS).Best,Dr. DesaiBeverly Hills Institute for Aesthetic Plastic SurgeryHarvard Educated, Beverly Hills & Miami Beach TrainedDouble Board-Certified, Double Fellowship-Trained Beverly Hills Plastic Surgeon

Answer: Breast implant removal and fat injection Thank you for your question and posting appropriate pictures. You seem to be a very good candidate for breast implant removal and fat grafting in the same operative procedure. I find you have the best amount of fat grafting take at the time of most expansion which is when you initially Remove them. I have done staged and immediate fat grafting and I find immediate with better take and shape. Of course you should meet with a board-certified plastic surgeon with extensive experience doing this.

Answer: Explant and The Bellesoma Lift The technique I recommend is explantation using The Bellesoma Method. The implants are removed, your breast tissue is reshaped creating upper pole fullness, elevated higher on the chest wall and more medial to increase your cleavage. Vertical scars are avoided, nipple sensation and the ability to breast feed are maintained, if not injured during your previous surgery. At the same time or later, fat transfers can be performed if additional volume is desired. Best Wishes,Gary Horndeski, M.D.

Answer: Implant removal Dear eirraceirrac, My preferred lift is a lollipop lift when a periareolar won't do (ie greater than 2cm lift required). The lollipop technique was created by a Canadian plastic surgeon named Elizabeth Hall-Findley and treats the breast as a three dimensional structure instead of two dimensions (anchor - lift). Most importantly it does not require the horizontal component scars that the anchor-lift requires and breasts appear much more natural, lifted, and less boxy. Lastly, the lift lasts longer because the lollipop doesn't rely on the skin to hold the breast up, its actually breast tissue thats being brought together to support the breast and hence improves longevity. If you are considering a surgery, I would suggest you to consult a board-certified plastic surgeon. Only after a thorough examination you will get more information and recommendations.
