I had a Septorhinoplasty in 2018 that involved a hump reduction and septal extension graft, which used the quadrangular cartilage from my septum as grafting material. In the 4 years since my surgery, my septum (or what remains) is deviated to the right, in a way that is disturbing to touch, and may also be causing a chronic whistling condition. Is a 'septal implant' surgery possible, which would involve separating the flaps and inserting cartilage back into the donor site?

The Silikon injections that were performed really complicates the future surgery. I would highly recommend returning to your original surgeon and discussing all of these issues.
Usually, we recommend to wait 6 months before getting a revision surgery, so it’s totally feasible. Your columella can be fixed without affecting your nose. I’ve personally performed those revision surgeries and patients are really happy with the results.
