Does Ultherapy cause apoptosis of healthy cells+healing by scar tissue? I'm a med resident, please explain scientifically

Ultherapy focuses ultrasonic waves to different levels under the skin to essentially cause micro burn spots in the tissue spaced far enough apart to leave intervening normal tissue. The marketing suggests that this stimulates new collagen that tightens the tissue and overlying skin.If you are an intellectually curious medical student, you should think of this as a burn injury. We know that all superficial skin injuries including burns heal by forming collagen scar tissue.  All wounds contract during the healing phase. Scar tissue is usually not the same as the original tissue.So we must ask some questions:Are we stimulating "new" healthy, elastic, collagen, or creating scar tissue?Does wound contracture occur at depth, or are we collapsing architecture by necrosis?Can we avoid damage with atrophy to fat pads in the treated areas of the face?Can we predict reliably which patients may benefit and which will not.I recently disposed of my Ultherapy machine at a $60,000 loss because I was unable to produce consistent, reliable, predictable, results.  Patients had some satisfaction during the early swelling phase, but tightening was rare and unpredictable.  I could not in good conscience provide an expensive service in which I could not predict or control the outcome. My machine went to Hong Kong.
