First realize that this area is off label for sculpsure use. If the applicators fit well in this area, you would be a candidate. I would see someone who performs a lot of these treatments to make sure you will obtain a good result. I would also consult with a cool sculpting specialist as well.
Sculpsure is a noninvasive 1060nm diode laser treatment that removes up to 25% of stubborn fat in the treated area in 25 minutes.It is FDA approved for the abdomen and flanks. It is safe if you get pregnant in the future. The body contouring effects are seen in as quick as six weeks with final results at 12 weeks. The results are permanent as long as you maintain your weight and health. The laser targets and destroys the fat cells only which are then naturally removed from the body. Since the laser does not affect the skin or muscle it can be used on all skin colors and there has been no reported incidents of burns when treated by a properly trained physician. It is protective since it alternates heating and cooling. It has four applicators that can be customized to treat the areas of stubborn fat. If a second treatment is needed we wait at least six weeks and usually closer to 12. If you notice lumps after (which I've never seen) I would recommend massage. You should also alert your surgeon. No compression garment required. There is no downtime and you can resume your normal activities right after the treatment. There may be some soreness in the area for a few days for which you can take ibuprofen or tylenol. A cold compress can also be used. Sculpsure is not used for skin tightening however some of our patients have seen this. If more skin tightening is needed we have combined this with thermi or Ulthera for a complete treatment. Sculpsure is recommended for stubborn areas of fat that are not being lost by diet and exercise. It is not a weight loss treatment Since I have both a coolsculpting and sculpsure I am working on protocols of utilizing both of these depending upon what areas need to be treated and the response to the treatment. I have used sculpsure on other areas other than the abdomen with very good results. I also use a Vectra 3D imaging system to try and show people what their treatment areas could look like. To get the contour back to your ankles, Sculpsure with a combination of radio-frequency should be successful. It is important to find a dedicated Sculpsure specialist for this. Unfortunately many plastic surgeons use Sculpsure as a "back burner" procedures preferring to focus on surgery. They often leave the procedure to technicians.There are clearly important techniques when using Sculpsure so seek out a center with extensive experience.
Treating your "cankles" with SculpSure is considered "off label", meaning it is not yet received FDA approval. But many surgeons are having good results. I have tried it on the ankles and it worked quite well, so I would try it and see if it helps, there are not many options out there for "cankles". CoolSculpting needs loose skin to suck into the treatment heads, so it won't work on the ankles.

SculpSure is certainly an option to treat "cankles." Although it would be considered an "off label" treatment, its rectangular applicators make for a good size (4cm x 6cm) and fit to attach to the area.It is also worth noting, the laser energy delivered by the SculpSure applicator fans out beneath the surface of the skin, so you will also see improvements in the centimeters surrounding the frame and applicator as well.SculpSure recommends two treatments for optimal results. In the end, the price of your SculpSure treatment may exceed the cost of liposuction. So, if you are not averse to surgery, you may want to consider it as an option as well.
Of course, it is difficult to accurately say without seeing just what type of volume of fat you have, but yes, SculpSure would be a very reasonable option. Because it has 4 separate handpieces, SculpSure allows the versatility to customize the placements to best suit your body. Again, depending on the volume, you may require multiple sessions, but the treatment is easy, with no downtime and minimal post-procedure effects, unlike some of the other non-invasive devices, like CoolSculpting.
