How can I reduce my nipple size?

My areolas get extremely big sometimes... it's strange because at times they will be so puffed out you can't even see the nipple anymore, and other times they will be so small (I like them when they're at this small size). The colder I am the smaller they seem to get.... is there anyway or surgery to permanently keep them like they are at they're smallest?

Thank you for sharing your question.  Unfortunately without a full series of photographs, or an in-person examination, it is difficult to offer definite advice as there are a number of different possibilities for your puffy nipples.  You may have a breast disorder known as tuberous breasts which is hallmarked by small, narrow breasts that have a high and tight breast crease.  You may simply also have enlarged areola.  In either case surgery can help correct your appearance.  See an ASPS board certified plastic surgeon in consultation.

Areola reduction  involves  circumferentially removing areola  skin  around a smaller areola design. This procedure can be done under local or general anesthesia. It is often  done as part of a brest augmenation/lift,  breast lift or breast reduction  procedure. If the areola are "puffy" (for example, as seen with constricted or tuberous breasts) some breast tissue excision may be helpful.

Patients should be aware that despite best efforts scarring will be visible and potentially abnormal ( possibly thick, raised and/or pigmented).  Some areola  asymmetry may also  be present after surgery.   Their  is  a small chance that  a patient may lose sensation.

As with all plastic surgical procedures, patients must weigh the potential benefits against the potential risks/complications  while making decisions.
