In my opinion you are definitely not a candidate for suction of the cheek area. Contour issues would be best help by fillers.On balance I really think you're being too self-critical. No treatment is certainly a legitimate choice.

Your question makes sense, however the big picture is loss of facial volume from above.This can be evened out with addition, rather than subtraction.Subtraction will likely cause skin laxity that will be troublesome.Best wishes.

You are likely seeing this because of your mid face volume deficiency.  Best to be evaluated in person. Good luck.

Over time we lose precious volume in our face as this is one of the most common areas I discuss with patients every day.  For this reason, I do not recommend liposuction of the face or cheek.  Our genetics play a role in many of our features and sometimes there is not always a perfect solution.  Everyone has different opinions, but if you have volume removed from your cheeks, I believe you would not be happy with that decision over time.  If you are at a stable weight, fillers such as hyaluronic acid may be able to soften the areas you are concerned with and maintain a natural improvement.

Each anatomic area generally looks the way it does InContext to the adjacent areas.Your low BMI and lack of midface volume may be contributing more to your peri oral mounds.I don't think you're going to find NEC solution to your situation.Midface volume restoration may be an option though I would be reluctant to do anything permanent or drastic.Midface volume can easily be restored using fillers.This can also give a slight lift to the corner of the mouth.Best,Mats Hagstrom M.D.
