How do I get rid of my "saddle bags" after massive weight loss and already having one tummy tuck?

There is only one way to correct surplus skin after massive weight loss in that is to surgically remove it. Skin tightening technology does not work. Consult with your plastic surgeon about extending the abdominoplasty incision to the back area.

Congrats on your weight loss. I know this can be a long and grueling journey but it is all worth it. It depends if there is excess fat or skin. If it's just fat then Lipo will suffice but if there is extra skin then you will require further skin excision. 

Great question for your plastic surgeon. If there is excess skin along the hip and buttock regions you might be a candidate for a circumferential body lift minus the tummy tuck which you've already had.

Congratulations on your weight loss! Often times when people loss a significant amount of weight, they get saggy and excess skin. Loose skin will fold over immobile skin which is held down (usually around the lower waist). A tummy tuck works well to get rid of the excess skin on the front, but sometimes you will have just as much skin extending towards your back and this may become even more noticeable after a tummy tuck. In cases where there is significant tissue on the sides and back, you may need a more significant surgery called a body lift which extends the tummy tuck all the way to the back and gets rid of the extra skin. Liposuction will not remove skin. Do not worry about bothering your plastic surgeon. Open communication is important for good, safe results and your plastic surgeon will be happy to hear how you are doing after your tummy tuck. Costs will range between physicians and regions and the exact procedure you will need. 

Congratulations on your weight loss. I cannot speak for your surgeon, but I would not consider it a bother if a patient of mine called with a question. Your situatuon is extremely common. The weight loss resulted in general skin deflation and sagging. Because our skin has so called Zones of Adherence where it is densely attached to the underlying muscles certain specific deformities are seen with weight loss. Most with unflattering names such as Saddle Bag or Riding Breeches deformity, Bat wing deformity or Turkey Gobbler Deformity. Without seeing photos much less examining you I cannot specifically examine you but if are like the thousands of weight loss patients I have seen, the Saddle Deformity you may be referring to can be corrected nicely with a Thigh/Butt Lift operation. Unfortunately, once you have lost large amounts of weight you end up with a lot of loose hanging skin. Non surgical means like liposuction will not help. You would need to remove it via a tummy tuck, lower body lift. This can be done anytime after your weight has stabilized.
