In my experience, patients in need of a monsplasty have had significant weight changes and so often are also in need of an abdominoplasty. I perform monsplasty in almost all large weight loss abdominoplasty surgeries in my practice.  In my experience, this fat is very resistant to liposuction only and usually requires liposuction, direct excision of fat and excision of skin. I would recommend finding a BSPS with extensive experience in large weight loss patients.
