Would Tretinoin prevent them further? Or should I start with Juvederm? How frequently and for how long would I need to take Juvederm? Are there any long term side effects? Thanks.

It is possible you might also benefit from some filler in the cheeks, which would add lift and further reduce these creases. Hyaluronic Acid fillers (Juvederm, Restylane, Versa) are wonderful fillers for the nasolabial folds. When done properly, they can provide instant gratification too. These fillers tend to last at least a year and if needed will use a small touchup thereafter.Tretinoin is for finer lines and wrinkles. In the photo above, you have more volume loss which will be best treated with volumization via fillers.

I recommend fillers to soften the nasolabial folds themselves and also lift around the mouth and cheeks to build underlying anatomical support. A fill face photo would have been helpful to determine the extent that age-related changes in the upper cheeks may be contributing to the smile lines. However, from what can be seen, the lines appear to be relatively superficial, and, as such, can be easily treated with a soft, cohesive filler, such as Belotero Balance--although plain Restylane and simple Juvederm may also be used. It wouldn't take much to give you the improvement you desire.  Individual factors, such as your genetics and how much sun exposure you get over time, will determine how frequently you will require treatment going forward.  And, as for how long you will need this treatment, this will depend upon how quickly science learns how to retard the aging process. Make sure that consult with a board certified aesthetic physician and be certain to ask to see his/her personal before and after photos of smile line filler treatments before agreeing to proceed. Thanks for your question and photo.  If this area of the face bothers you a lot, you certainly could see a board certified dermatologist or plastic surgeon for a consultation and see if they would recommend Juvederm or one of the other hyaluronic acid fillers (there are many).  Your physician will be able to examine your face at rest, and also in animation (or movement, i.e. facial expressions) to evaluate how to best treat you.  One thing you want to avoid is just focusing on one part of the face - which sometimes will look unusual if just that area is treated.  Some people do better with some filler in the upper cheeks, which can lift some of the heaviness off the nasolabial folds for a natural, subtle improvement.  It is important to get a full evaluation to make a recommendation.  Please see an expert, and best of luck!

Hello! Yes, you will need a filler such as Juvederm to correct your folds. Tretinoin is wonderful for skin rejuvenation, but it does not fill creases or correct volume loss. Thankfully, since you are young, you would not need the amount of filler that many older patients require. I would estimate 1-2 syringes for your initial correction, and then 1-2 yearly for maintenance.
