In some B&A photos of cheek implants, I notice that the patient's nose (the nostrils especially) looks a bit wider in the after photo. This would make sense, being that the cheek implants do stretch the skin to some degree. Is this an unavoidable side effect? Or does it come down to implant size and the surgeon's skill?

First, you have to understand that there are different types of facial implants that may be grouped under "cheek implant" (eg. pyriform, zygomatic, orbital rim, or combination). These devices are use to augment the bony structure but also do have a significant effect on the soft tissue. Size of the implant, positioning, extend of disection of the pocket can all have a profound effect on the final "soft tissue" result. Good communication between the patient expectation and your surgeon will ensure that everyone is on the wavelenght. Clearly, experience of a surgeon always does have some influence but an understanding of what can be accomplished with cheek implants is as important.
