I have a round, shapeless face with skin that hangs in folds and jowls. I had a jaw extension surgery as a teenager for orthodontic reasons, but I think the surgeons ought to have put in a chin implant to finish the job. The nasolabial folds are even worse when I smile. Would a chin implant tighten my face, lengthen it and give it shape?

This can be done either under local anesthia or general its just a very small incision under the chin that you will not see once healed and the implant is placed under there. You will need to wear the chin strap afterwards during the healing process. You should go an talk to a plastic surgeon and see what is the best way to do this.

Your situation is a bit more complex than most since you already had orthognathic surgery and also you require a bit of horizontal as well as vertical extension. I am assuming your dental occlusion or bite are fine now. Looking at your pictures you could stand a bit more projection in the chin. To vertically augment one can use a vertical extension chin implant as well as a procedure called sliding genioplasty. In a case where you have already had jaw surgery however, this technique becomes a bit more difficult. I would seek a board-certified surgeon experienced in this surgery who can meet you in person and talk about all your options. I hope this helps!
