BBL 11 years ago - Is this scar tissue ?

Hi! I had a BBL 11 years ago & shortly after i had a fist sized seroma. It was drained by my surgeon, but after the area was a bit loose a has a small sag on lower butt, its definitely slightly bigger looks than my left side. When I squeeze it it feels strange almost like no feeling unless I squeeze really hard. I was massaging the area for the first time really and it was a muscle like cramping , i got really scared went to the er and they gave me an ultrasound. The doctor cleared me saying no seroma or major concerns. I know what im feeling isn’t normal , can someone please help me understand this ultrasound & if an MRI is necessary ?

I understand your concern. However, without a proper assessment, it would be difficult to determine what is wrong. It is best that you visit your plastic surgeon for further assessment or ask for a second opinion. Only after a thorough examination, you can get proper recommendations and advice.

Regarding the ongoing discomfort and contour changes 11 years post-BBL, it is reassuring that the emergency room ultrasound ruled out active fluid collections or acute seromas. The localized altered sensation and muscle-like cramping you experienced during massage can occur due to long-term deep tissue remodeling, historical scar tissue formation, or underlying muscle fascia tension following the previous seroma drainage. While ultrasound is an effective base line modality for identifying fluid, an MRI would offer a significantly higher diagnostic yield for evaluating deep soft tissue layers, fat graft survival, and structural asymmetry. I recommend a clinical evaluation to perform a comprehensive physical assessment, which will allow us to correlate your symptoms with the ultrasound findings and determine if advanced imaging is indicated to optimize your treatment plan.

A firm or strange-feeling area years after a BBL can come from several things, including scar tissue around an old seroma cavity, fat necrosis, soft-tissue laxity, or nerve/sensation changes. I would not try to interpret ultrasound images online. If the ER ultrasound showed no current seroma or major concern, that is reassuring, but it does not explain every contour or sensation issue.
Because this has changed enough to worry you, the next step is an in-person exam with your original surgeon or a board-certified plastic surgeon. MRI is not always necessary, but it can be considered if the exam finds a deep mass, persistent pain, unclear ultrasound findings, or progressive asymmetry. Rapid swelling, redness, fever, drainage, or severe worsening pain should be checked urgently.
