Dear doctors, I had teosyal redensity 2 in my tear trough 3 years ago. It caused discoloration due to too superficial injection/tyndall effect. I've been waiting since that day for a self-dissolving but I still have the problem. Will hyaluronidase be effective on my filler? I'm afraid it could only dissolve my natural hyaluronic acid leaving the filler and making situation worse.
Answer: Dissolving tear trough filler Thank you for your question. Hyaluronidase is more effective on HA filler than it is on natural HA chains found in the skin. Usually any breaking down of natural HA will be replenished by the body. It can take more than one session of dissolving in order for filler to fully dissolve. But the likelihood of the hyaluronidase making the situation worse or not affecting the filler at all while causing issues with your natural HA is slim to none. I would recommend consulting with an experienced provider to address all of your concerns prior to booking a procedure. Best of luck

Answer: Recommend doing filler reversal with different technique Filler migration, swelling, and granulomas can cause significant discomfort and cosmetic ramifications. Our office specializes in the use of enzymes and filler reversal techniques without hyaluronidase to help improve the results of botched cosmetic fillers, whether caused by Juvederm, Restylane, or foreign fillers. Our plastic surgery institute provides expertise in filler removal. We begin by examining the facial fillers and assessing the likelihood of fluctuance or liquid volumization. If appropriate, we use Hylenex, or other filler reversal agents to reduce swelling and filler migration. Hylenex can be targeted into the areas of concern using multiple layered injection techniques. If the filler does not completely resolve, then additional concentrations may be employed. The technique has the potential to reduce the filler and may slightly impact normal hyaluronic acid. Treatments are repeated and results can improve immediately and with time. Best, Dr. Karamanoukian Realself100 Surgeon

Answer: Hyaluronidase Works Well For Dissolving Even Long-Standing Hyaluronic Acid Filler I have both experience with Teosyal Redensity II for the tear troughs (having you used it for years in my satellite practice abroad) and I have extensive experience in the use of hyaluronidase for dissolving unwanted filler. The short answer to your question is that hyaluronidase can be used even several years later with good results for dissolving unwanted filler, which, under the eyes, may actually last for several years (I have had a few patients whose results under the eyes lasted between three and seven years). So long as the swelling under the eyes is not from lymphatic edema, hyaluronidase injected in the appropriate amounts and concentrations should improve the condition. I have long asserted that hyaluronidase can affect normal surrounding normal tissue, as well, so it is imperative that you consult with an experienced board certified cosmetic dermatologist or other core aesthetic physician. Should this happen a small amount of hyaluronic acid filler properly injected should readily correct the problem. I am no fan of autologous fat injections. In fact, I gave up using fat injections years ago for a number of reasons. For one, it is a relatively expensive, two-step, minimally invasive technique--one wound to withdraw the fat with a large cannula from the hips, belly or buttocks, and a second procedure with a large cannula to inject it under the eyes. There is downtime associated with both procedures. And, if there are lumps or other complications, there is no enzyme to dissolve the injected fat. Perhaps at least as troubling is the fact that the injected fat retains the properties of the region from which it is taken. This means that as one ages, fat taken from the midriff region can develop a "midriff bulge" in the injected location, just as it would in its original site. Make sure you are consulting with an experienced injector. Best of luck.

Answer: Hyaluronidase for fillers It can take many years for hyaluronic acid filler to dissolve on its own in the tear trough area. If there is still filler present, hyaluronidase should help dissolve it. Additional filler may be necessary afterwards but placed differently.
