im 28 & have congenital malar bags, prominent nasolabial folds, & flat/sunken mid face. im trying to figure out my options. if i got malar bag surgery, could i get facial fat transfer after? does the surgery solve the root cause or only the appearance? also would i benefit from a mid facelift/full face lift at this point? my face appears prematurely saggy and hollow. id like to avoid filler if possible.

In my opinion, the best treatment for nasal labial folds is with fillers. Fillers are precise, predictable, and quite forgiving. Fat transfer is not particularly good. Forgiving volume in very small, precise areas. It works better for giving diffuse overall volume. Facelift is not a good...
Have you had any radiofrequency? That can sometimes melt fat and make one look older. Yes, fat transfer can restore faces to an earlier age. Do be sure to see someone with a lot of experience, since FFG is more of an art than science.
It has been our experience in over 25 years of use of fat grafts for periorbital rejuvenation that graft take is around 30% in average. This means also that fat grafts do not take equally in all areas. Secondary grafting is done in probably 20-30% of patients 8-12 months after first procedure....
