Implant exchange or removal to treat capsular contracture grade 4?

I got silicone implants(below muscle) and a lift in 2018. Now I have severe capsular contracture-I tried aspen just under a year ago and had mild improvement but the problem is persisting. I'm weighing the options of en bloc capsulectomy with a lift and reduction in nipple size (since they've stretched now) I'm concerned about death or loss of sensitivity in my nipples because it took years for it to recover... Or replacing implants with risk of Cap. contr. and future surgery :(
