I had a lower blepheroplasty with fat repositioning completed. At the 3-4 week mark, the results were perfect with a smooth contour. I went for drinks that weekend and the next morning the bags appeared to return, which I just thought was swelling from the alcohol. But 6 months post surgery and the bags remain. Overall, there is definitely an improvement in the hollow area compared to before surgery. But the bag that returned is annoying. What are my options? Could steroid injections help?

Answer: Without physically examining you, it is speculating to explain this fullness. Not all fullness is caused by eyelid fat. Even if this is fat (and it may be) the answer is most definitely not injecting this volume with steroids. Some fat should live here. The injected steroid is dumb. It will harm fat in an uncontrolled way. The steroid can also atrophy the skin and connective tissue. For these reasons, it is not just a bad idea to use steroid to address this, it is a terrible idea. You may from volume at the top of the cheek to help blend these areas. This could be done with hyaluronic acid filler, which has the advantage of being an office based service. The left lower eyelid may benefit from revisional surgery.

Answer: Residual "Eye Bag" after Blepharoplasty I assume you had a transconjunctival procedure since I see no external scar. What you have is why I stopped doing that procedure and even more aggressive open blepharoplasties, especially when the medial orbital ligament is separate from the lateral one like yours. It is almost impossible to move enough fat to correct the volume loss and the ligament reforms. What you need is volume replacement below the ligament(s). This requires filler or fat. Juvederm Voluma is my present choice since it does an excellent job and can last over 5 years in this location. Make sure you see a true expert as the average injector uses the wrong filler in the wrong place.
