want to explant not sure what to expect or to lift. 31 yo, no kids(yet), 115-120lbs, active. Implants 10 yrs old, put in during reduction/lift. was 30ddd, to 32c. gained weight after, lost weight, implants changed, sagged. want to remove, had 1 consult,2 more planned. not sure if I should lift? desired b cup. not sure how much of my tissue is left vs. the implant? please advise! thank you!
Answer: Lift + Explant Important to note how many cc implants you have, a large implant of 700 cc versus a small 300 cc implant will have different outcomes. For someone who is 31, a lift would not be recommended in general, given the fact majority of implants are placed BELOW the muscle. Whereas a lift is entirely completed ABOVE the muscle. Given my practice, a lot of patients do not need an unnecessary lift operation, which is extremely invasive.

Answer: Hard to know answers to above without an exam or pictures A mastopexy/lift will reposition nipples in a more aesthetically pleasing position. When you take out your implants you will lose a significant volume but only with a physical exam can I help guide you regarding your native breast tissue. If you have a significant residual breast tissue volume then a board certified plastic surgeon can help guide you toward the surgical approach best suited for you. There are benefits to a superior medial pedicel technique to maintain superior pole (cleavage) fullness assuming your lift needed is safe to achieve those results. Alternatively, if you have less breast tissue a plan can be catered to your needs that includes fat grafting. The decision to explant should be based on your aesthetic goals and desire to maintain the implant and their risks. There is also a cost difference that I am putting aside. Hope that helps. Your surgeon will be able to estimate how much of your natural tissue may be left from an in person evaluation. Any information you are able to provide about your implants (size, etc) will be helpful.

Answer: Need to see what you look like! Photos or examination are definitely needed to better answer your question. Based on your description, it sounds like a revision lift would be necessary. Usually this can be done at the time of the explant, but sometimes if you don't have much tissue it needs to be done in a second operation.

Answer: Lift with removal? Hard to give you a specific answer without seeing photos but in general most patients will need a lift after implant removal if they have gone through the changes in weight etc that you have and have had their implants for ten years. If you have saline implants it is better to rupture them before having them removed because it will give you an idea how much volume you will have after removal and it also makes the lift safer to perform. If you have silicone implants then you can feel your breasts and see what is implant and what is breast tissue by pushing the implant towards your chest--hard to describe but it's quite easy to do--and that way you can get an estimation of the volume of your own tissue.
