Bulbous round tip with no definition - how can I improve this? (Photos)

Based on your photos, you appear to have a nasal tip appearance that sticks out a bit too far, and this is associated with droopiness. I can appreciate wideness, and some asymmetries, on front view. If you have severe dissatisfaction with the appearance of your nose, you should consider meeting with several reputable revision rhinoplasty specialists so you could see what to expect from additional surgery. It is a very challenging problem to give definition to a bulbous tip with thick sebaceous skin. Even if more cartilage is removed the resulting swelling will lead to more scar tissue and the tip will end up looking the same over time or even worst. It is best to leave it alone.

Hello and thank you for your question. Based on your photograph, you may benefit from a tip refining rhinoplasty. Your surgeon can accomplish this by trimming, suturing, and reshaping the cartilage in your lower nose. Fascial grafts may also be used to help improve tip refinement. With good surgical technique, you could have an outstanding result. I recommend that you seek consultation with a qualified board-certified rhinoplasty surgeon who can evaluate you in person.

Thank you Stan for your question. One other view that would be very helpful in terms of assessing the nasal tip is a worm's eye view looking at the tip from below. In looking at the pictures you send you had a very nice result from your previous rhinoplasty but according to you you are unhappy with the tip. Certainly a tip revision could be performed possibly adding a small shield cartilage graft to accentuate the tip and this might be 1 of the easiest ways to go to give better definition. Certainly without feeling the tip and actually assessing the nose in person it is difficult to say but this is something that is not uncommon and revision occurs in about 10% of patients Nationwide after a rhinoplasty.
