Question

I had my first rhinoplasty in 2022. 1.5 years later I had a revision as the cartiledge was caving in on one side making it hard to breathe. Now the tip is bulbous and my nostrils still flared and my nose still feels huge. Do I need another revision? I’m going next week to my doctors to get a steroid shot in the tip but I literally hate how my nose looks:(

Of course, all I have to go on right now is these two frontal views (lots of other angles would help a lot), but it sure looks as though the size of the tip is due to the size and strength of the tip cartilages below the skin. If that's the case, then yes, you'd need a revision to make things a lot better, and boy, I wouldn't have the same surgeon do the revision.

With a good physical exam in the hands of a real expert, the doctor would probably be able to determine what's making the tip look big (and maybe, from the other views, it might sit low as well), and what needs to be done to make your nose more like your original goal.

Hello and thank you for your question. Based on your photograph, you may benefit from a tip refining revision  rhinoplasty. Your surgeon can accomplish this by trimming, suturing, and reshaping the cartilage in your lower nose. Fascial grafts may also be used to help improve tip refinement. This can also be designed to help improve the nostril appearance.  With good surgical technique, you could have an outstanding result. I recommend that you seek consultation with a qualified board-certified rhinoplasty surgeon who can evaluate you in person.

In general, we do not advocate steroid injections to treat a wide nasal appearance. Since you are 1.5 years postoperative, you should be fairly close to a final result. If you are satisfied with the opinion of your surgeon, you may wish to consider getting a second opinion from another reputable rhinoplasty specialist so you can see what might be best for you moving forward.
