Breast lift with implant surgery performed to address sagging breasts and nipple placement after having children. Prior bra size to having children was 32D. After, 34 B/C. Hope was to restore fullness with implant and achieve D cup, however breast shape is full but still low, nipples are also still sagging, one is irregular. One breast is also visibly flatter, the other full, although not as noticeable in the photo. Donut technique was done around the nipple with horizontal incision at crease.

It is important to allow the implants to settle and I routinely recommend waiting at least six months prior to undergoing a revisional procedure. The donut technique ( periareolar mastopexy) has the advantage of less scarring but is more limited in the ability to elevate the nipple areolar complex when compared to techniques that utilize either a vertical scar ( circumvertical mastopexy) or an inverted T scar. If the appropriate amount of time has passed and you are still unhappy with the appearance of your breasts, I would recommend a revision with a circumvertical mastopexy with or without an extension of the incision along the inframammary fold. This technique will elevate the nipple areolar complex and remove volume in the lower portion of breast, improving nipple areolar position and breast shape. Good luck.

Answer: Lift The doughnut lift is limited in what it can do and it cannot do all that you need. You need the vertical lift. Your areolae can be lifted more and the excess skin removed and breast shaped with the vertical lift.

Answer: Explant w/Bellesoma Method At this time, I recommend explantation and lift using The Bellesoma Method. The implants are removed, your breast tissue is reshaped creating upper pole fullness, elevated higher on the chest wall and more medial to increase your cleavage. Vertical scars are avoided, nipple sensation (in 95%) and the ability to breast feed are maintained if not injured during your previous surgery. Over many years, I developed a technique called "Reductive Augmentation" in which an internal bra-capsulorrhaphy, mastopexy - lift, and removal of inferior pole breast tissue is performed. This procedure is particularly suitable for patients who have a lot of inferior pole breast tissue. I have treated many patients with very similar breast issues as yours. If ever used, it can produce only a 1-2cm nipple elevation, which is exactly what you got. What you needed was more of a 6-7cm elevation of your nipple. You need a full formal breast lift, using either the vertical (lollilop), or anchor techniques. Good luck!
