I had breast augmentation in December 2020 (Saline 250 ccs). I wanted a very modest natural breast augmentation tear drop shape. My concern with the results is that there is virtually NO underboob/inner crease. I previously did not have a crease either. Is this possible to create as part of a revision surgery or can I not achieve this because of anatomy (understand my breasts are spread apart). Would a larger implant help? I wouldn’t like that much more projection just a natural look.

Yes, your implants are sitting a little too low. Your lower pocket can be tighten and raise the implants back to their normal position. Please do not go bigger, or you will be back where you started. You might even consider going a little smaller.
An examination is needed to be sure, but it appears that your capsule has torn near your breast bone and the implant is poking through. This can be fixed surgically.
