How to fix my long face? (photos)

Question

I feel I have a shapeless and undefined face. I have "jowls" that cast shadows near my mouth and flat cheekbones as well as a long face and large nose with a deviated septum. What can be improved to give it a more feminine look?

There are a few ways to approach this that can help soften your facial features.  Fat transfer is an excellent procedure that can add soft tissue volume and add dimensions to a "shapeless" face.  You can improve your mid face by adding more volume to your anteromedial cheeks and the lateral cheekbone areas.  This would also make your lower face appear less heavy.  You would also benefit from a rhinoplasty that can correct the deviated septum, shorten your nasal length and refine the nasal tip.  Last but not least, and probably the easiest thing to do, is to consider getting a hairstyle with bangs and that would make your face appear less long.  Hope that helps!

I think the keys to making your face appear more defined and feminine are nuance and subtlety.  Certainly, the greatest immediate impact would be achieved with a reduction rhinoplasty that narrowed and refined your bifid nasal tip, as well as shortened its projection, in addition to a conservative narrowing and shaping of your nasal bridge.  At the same time, fat transfer to the cheeks could be performed to increase both their anterior projection and the bi-zygomatic width, which would balance the perceived excess vertical length of your face, particularly in the middle third.  Some of that fat could also be placed in the upper lip, which is a bit out of balance with your very full lower lip.  While none of these maneuvers in isolation will dramatically change your facial appearance, performed in concert, their cumulative effect will be to a create a subtle but attractive feminization of your facial features that will obviate the need for more aggressive facial surgery.

I you have some excess fat that is giving you an appearance around your mouth that you don't like but not true jowls.  Adding volume to your cheek bones with an injection of filler maybe the prudent way to start to improve the middle third of your face.  The rhinoplasty will also give you a change.  All of this is going to change your appearance and it needs to be planned and discussed carefully with you.
