Columella looks wide. What can I do? (Photo)

The relationship of your columella to alar is not harmonious. You have a widen columella and collapse of your alar (pinched tip). Do you have nasal breathing problems as well? This could indicated dynamic external nasal valve collapse as well?
Generally this deformity can be corrected with surgery - revision rhinoplasty. You would need cartilage harvest from your ear to reinforce the alars (rim grafts or subalar strut grafts) and revision of the columella incision and reduction of the columella width.
Seek out surgeons with an interest in revision rhinoplasty. Best of luck.

Thank you for your question.The width of your columella is related to both the amount of skin and the width of the underlying cartilage. In addition, it appears relatively wide owing to how your nostrils are small due to collapse of your alar cartilages.Reduction/modification of the columellar skin and underlying cartilage, as well as adding cartilage from your ear or other site to help lift your alar rims would be best to help address your concerns. Best of luck!

you appear to have partially collapsed/notched alar margins and widened footplates of your medial crura.... the ala can be addressed with alar rim grafts or alar batton grafts.... the columella can be addressed by manipulating the medial cruragood luck
