It is a simple procedure to pull back the nasolabial angle and the columella under local anesthesia. As long it is done conservatively it will work well. The recovery should be quick. RegardsDr. J

Your columella may appear pushed down due to an elongated nasal septum, or if your surgeon placed a 'septal extension graft'. If your issue requires septal shortening, this may be a relatively easy fix. If you have concerns, get an opinion from another rhinoplasty specialist. Thank you. Dr Joseph

Your columella can be elevated under just local anesthesia if you desire. This is a relatively straight forward procedure with minimal risk and discomfort.

If you want to be sure, go for a second opinion with a surgeon who specializes in rhinoplasty and revision rhinoplasty.  Based upon your photos, it appears this should be able to be addressed and can be done under local anesthesia. Best,Dr. S.
