A combination approach would give the most improvement. Deep wrinkles and smile lines can be improved with a combination of Botox and/or fillers for the lines themselves and to build anatomical support with Sculptra for collagen building. Ultherapy is a great skin tightening device, but works best in combination with other treatments like J Plasma, RF treatments, lasers, microneedling/PRP, salt facials, Cosmelan for skin tightening and improve skin tone. Additional photographs are needed but it is possible that you would benefit from a lip lift. A lip lift does 4 things: shortens the distance between your nose and upper lip, increases the mucosa of the upper lip, increases the incisor show and pulls up the corners. There are a number of reasons as to why a person can have a horizontal lip fold. In many cases lack of philtral columns can cause this wrinkle and in other cases it's the muscle of the upper lip. In one instance a filler should correct the issue and in the next a small amount of botox will most likely result in significant improvement. When you smile the upper lip is pulled upward by the action of cheek, not lip muscles. The upper lip then crashes into the nose and makes a wrinkle. I’ve never tried doing botox there.....it never made sense as it paralyzes the muscle that isn’t contracting when smiling. Paralyzing that muscle also would keep the upper lip from puckering, which is what the lip muscle does.I don’t have a solution for this problem.

Your surgeon has done an admirable job. Likely you are holding on to previous feelings from earlier. From an outsider's perspective, you look really good. Fillers are the only option if you want to go for "perfect", but they are temporary and costly for someone your age.
