From the photo, you are a good candidate for a necklift. A necklift can improve the underlying structures of the neck and allow the neck to lie flatter. You may need additional procedures on the skin afterwards like Fraxel and/or Ultherapy to further tighten in. Be sure to see a facial specialist for consultation.

Although further assessment is needed, possible procedures to consider would be a micro neck lift and a deep neck liposuction. The micro neck lift would address the extra sagging skin on the neck and allow for a youthful rejuvenated look. The deep neck liposuction would address the excess fat underneath the muscle and lessen the heavy appearance of the neck. It is certainly possible to perform an isolated necklift to tighten neck skin and muscles, as well as remove any extra fat. The only reason to add the cheek and upper facelift to the mix is if there is noticeable loose skin there. Based on your photo, I think the necklift should help smooth and firm your neck.

​A combination of non-surgical options like Kybella, PDO NovaThreads, Ultherapy and Thermage would do well in combination for skin tightening for the neck. VASER Liposuction (with ThermiRF, Renuvion and InMode FaceTite) is a great alternative for contouring and tightening the chin and neck and can be combined with a neck lift at the same time, if you're a candidate. Much more information is needed, such as a full set of facial and neck photographs to make a determination about how best to proceed. In our practice, a necklift does not remove any skin whatsoever, it just removes fat deposits above and below the platysma muscle which also includes a platysma- plasty. If you have loose and inelastic skin in the neck, a lower face and neck lift is going to be required to tighten the loose facial and neck skin.

I agree you need open full neck lift with muscle plication. Most boarded PSs offer this type of surgery. Expect fees in the $10,000+++ ranges.
