Can I get a better result? (Photos)

Good morning!I perform over 200 XL augmentations a year, and I do believe it is possible to achieve your goal size and look. To achieve the size and look of your first goal photo (with necklace) will take 800cc UHP, and the girl by the window and jogging is bigger so you would need more volume to achieve that size. Since silicone only goes up to 800cc currently in the U.S. it will mean a switch to overfilled saline, probably in the 900-1000cc range. And your last goal photo set which are clinical side by side photos this would be back in the 800cc UHP range.I have attached a link to my XL Breast Augmentation photo gallery for your review, which shows real patients and lists how many cc's I placed- hope this helps!

The pictures show bottoming out over the right side and very tight breasts. The cleavage is at the border of becoming symmastia. I would recommend repair of the right capsule and possible fat grafting to buildup your soft tissue coverage of the implants. Your desired pictures show breasts with more fat to start with and just going with larger implants has high risks. Your implants are above the muscle and there is a limit how much your breast can expand.

I would NOT recommend "anatomic" implants as you WANT upper pole rounded fullness. Your right inframammary fold needs to be higher and you need a much fuller implant -which likely means saline unless you moderate your size goals somewhat. Staying subglandular versus switching to submuscular is a conversation you will need to have with your surgeon as their are potential trade-offs that need to be discussed.

Based on your description of goals and after viewing photographs, I think that it is possible to achieve your desired outcome.  A combination of capsulorraphy (adjustment of the breast implant capsules) along with selection of appropriate breast implant size/profile (possibly a larger ultra high-profile silicone gel breast implant) will likely be necessary to achieve your goals.  In my practice, I would not recommend the use of anatomical breast implants or conversion to the sub muscular space, at this point. Best wishes.
