I am 37 and was in a car accident 3 years ago that damaged my left nostril, among other cosmetic injuries to my face. A year after the accident I had a septoplasty (to fix a deviated septum) & ear cartilage graft to my nostril to try to fix the asymmetry. I am overall pleased with the surgery but now I'm self conscious about a part of my face that I used to really like. I've been told by my plastic surgeon to leave it alone but I can't help but wonder if there's a way to achieve better symmetry.

In general, the chances of infection from rhinoplasty with or without alar base reduction of very small given the great blood supply to the face. Alar base/sill reductions are very safe in experienced hands. The primary concern would be the obvious appearance of a scar which is why I recommend...
It is normal that nose opens more when you are smiling due to attachment of muscles to the allar base. There is also muscle that attaches to the columella of the nose and pulls tip down when smiling. This one can be detached during rhinoplasty or it can be procedure for itself. Hope this helps.
