Fatty pad inner eye, after bleph - will this be hard to fix?

My doctor didn't seem to address this with initial bleph - only the upper lid. Now this really bothers me... especially the right eye. Eye shape has changed and looks more "beady" How straight forward is this to fix and will there be a scar since the incision won't be in the crease?

Thank you for your photo.  The small residual fat pocket that you have can easily be revised under local anesthesia using the original incision from your upper blepharoplasty.  Only a small amount of residual fat would need to be trimmed and your old incision would be as well camouflage as it is now.  Care must be taken to not remove too much fat as hollowing of the upper eyelids tends to cause an aged appearance. Best wishes.

It appears the right upper lid nasal fat pad was not appropriately reduced. This is a relatively straight forward procedure to fix. A lid crease incision is again used (ie the same incision site used for your upper lid blepharoplasty) and the fat is appropriately excised or transposed. Best to see an eyelid expert.

Removing some additional tissue, fat and possibly a slight amount of muscle, to sculpt this area is straight-forward and can be achieved through the same scar that was initially placed.

Looking at your pictures, you could consider some additional skin removal at the same time as well, if desired.

If revision surgery doesn't interest you, CO2 laser resurfacing can help to tighten the tissues. It might not resolve the degree of fullness that is concerning you, but it remains as a surgical alternative.
