Hi I am 52 and ready to go through Septoplasty, Turbinate reduction, Ethmoidectomy, and Maxillary Antrostomy with tissue removal. No packing or splints will be used and this is bilateral. Im so congested now before surgery with 2 obstructions. Will I have even a little air flow through my nose right after surgery? The next day? I can deal with pain and congestion. I just need a little air flow. Thank you.

Answer: Your breathing will probably be improved very soon after surgery.
It's possible this is a caudal septal deviation, which would not usually be addressed in a traditional septoplasty (the first one you had) and may not have been addressed either in your 2nd surgery... but it could also be due to the position of your lower lateral cartilage/medial crural...
This could be from cartilage damage or cartilage infection or surrounding soft tissue infection. You will need to see someone to evaluate the actual cause. It can probably be cut out and corrected surgically.

The rhinoplasty is an intricate, complicated operation, and should not be done at the same time as sinus surgery. A septoplasty is often part of a rhinoplasty, so combining those two is perfectly okay.
Do you know whether you *must* have the sinus cysts corrected? If that's an optional...
