I had a nose job to remove the hump. I specifically asked to not shorten my nose. I had two consultations and made it clear that I likedmyfront view and didn’t want any changes to the nose lenth or tip rotation. My surgeon is saying that at one year healing mark my nose is going to look like a morth that he designed/was discussed. I just don’t see how it can get from my current state to that. What are your opinions? Also, what are my options on how to fix it?
Answer: Upturned / Shorter Nose Hi there! Sorry that you're experiencing trouble after your surgery. If a caudal septal extension graft isn't placed, the nose doesn't often have an area to be anchored off of. This graft allows for the foundation to be set. Upturned and shorter noses indicate less than ideal amounts of cartilage at the front of the septum and too aggressive of repositioning of the nose. A revision procedure would be needed to counterrotate the tip and position it in a stable spot. I would seek consultation with several Facial Plastic Surgeons to learn more. Hope this helps!

Answer: Rhinoplasty recovery: what you should know Thank you for your question and for providing photos. It’s important to know how long ago your rhinoplasty procedure was performed to make a proper assessment. It’s not uncommon for surgeons to over-rotate the nose tip upward since it generally drops as the nose begins to heal. Without this step, it would be possible for the nasal tip to droop downward and produce an unfavorable result. Since your nose will be in a state of recovery until at least one year after rhinoplasty, it would be best to wait until this time to consider a revision. If you decide to go forward with another surgery at that time, the tip can be addressed to create a feminine, natural-looking result that complements your other facial features. Please keep in mind that each subsequent nose surgery becomes more complex due to loss of structural support, scar tissue, and other factors. The importance of experience, expertise, and artistry cannot be overstated. I recommend scheduling an in-person consultation with an experienced, board-certified facial plastic surgeon, with expertise in rhinoplasty techniques. They will be able to review your surgical history, answer any questions you have, and help you determine the best course of action.

Answer: I’m not sure you need a revision. Your nose may be shorter and your tip more rotated than what was morphed, but the feasibility of undergoing revision rhinoplasty with “septal extension grafts” would require an examination of your situation. Consider asking your question with before and after photos. I’m sorry you’re not happy.

Answer: What are my options to fix it? Hello @Nurturing260458, thank you for your question. Remember that the nose is a unity and it must be treated as such to achieve harmony. That said, it is important to understand what may have caused the result of the rhinoplasty procedure to not meet expectations. An evaluation concerning several factor including the technique used during the surgery is necessary in order to know what can specialist can do. Best wishes! Alan Gonzalez MD, FACS.
